PDA first appeared in a journal article in 2003. Elizabeth Newson described PDA as sitting under the umbrella of Pervasive Developmental Disorder. Pathological Demand Avoidance (PDA) is one of several Autistic Spectrum Conditions (also known as ASD, Autistic Spectrum Disorder). The central difficulty for people with PDA is their avoidance of the everyday demands made by other people, due to their high anxiety levels when they feel that they are not in control. Pathological (or extreme) demand avoidance is a term sometimes applied to complex behaviours in children within—or beyond—autism spectrum disorder. The use of pathological demand avoidance as a diagnosis has, at times, led to altered referral practice and misunderstandings between professionals and the families of patients.

The ‘Extreme Demand Avoidance Questionnaire’ (EDA-Q) is a measure designed to quantify traits of extreme/‘pathological’ demand avoidance in children aged 5-17, on the basis of parent or teacher-report. The questionnaire was developed to ensure that PDA traits were being measured in a consistent way for research purposes. O’Nions and co-workers (2014) have developed a “trait measure” for PDA (“the EDA-Q”), a measure that appears to hold considerable promise for research, and clinical practice. The instrument that they have developed is a 26-item parent questionnaire that appears to be easy to use and with preliminary good-excellent psychometric properties. This study reports the development and preliminary validation of the 'Extreme Demand Avoidance Questionnaire' (EDA-Q), designed to quantify PDA traits based on parent-reported information.

The observer-rated EDA Questionnaire (EDA-Q) for children was adapted as an adult self-report (EDA-QA), and tested in relation to personality and the short-form Autism Screening Questionnaire (ASQ). The ‘Extreme Demand Avoidance Questionnaire’ (EDA-Q) was developed to measure behaviours reported in clinical accounts of extreme/‘pathological’ demand avoidance (PDA). The original EDA-Q for children showed good sensitivity and specificity for the construct, and had an internal reliability (Cronbach’s alpha) of 0.93. It was concluded that the EDA-Q was a potentially useful research tool (rather than a diagnostic tool) for identifying the PDA profile. Although aimed at professionals, this paper would be useful to parents who may have experience of the EDA-Q and would like to research its validity and origins.
